About the Role
As a Transitions of Care Pharmacist, you will be responsible for conducting comprehensive medication reviews, engaging in patient-centered medication therapy management, and collaborating with healthcare providers to optimize medication regimens. You will also assist with medication reconciliation, provide patient counseling, and support interdisciplinary care planning.
About You
Required:
- Bachelor's Degree in Pharmacy or PharmD from an ACPE accredited pharmacy school.
- A minimum of one year of work experience, preferably in an acute care setting.
- Strong clinical therapeutic skills and knowledge of appropriate medication dosing in geriatric and adult populations.
- Excellent organizational and communication skills.
- Proficient computer skills including Google Suite and/or Microsoft Office.
Preferred:
- Two years of work experience.
- Pharmacy Residency (PGY-1).
- Experience with the 5 Core Elements of Medication Therapy Management Services.
- Strong verbal and written communication skills.
- EPIC experience.
