Lumiere Children's Therapy Chicago is seeking a Licensed Clinical Social Worker (SW) to join their team in Chicago, IL. The role involves coordinating and providing social services for patients of all ages in an interdisciplinary setting.
About the Role
As a Licensed Clinical Social Worker, you will develop and maintain the social service program, conduct home visits to assess patients' psychosocial status, communicate with physicians about patient progress, and work closely with the Lumiere team to develop treatment plans. You will also represent the agency in community resource groups and maintain knowledge of Medicare, Medicaid, and other reimbursement sources.
About You
Required:
- Master's degree in social work.
- Licensed Clinical Social Worker in the State of Illinois.
- At least one year of social work experience in a health care setting with home care experience preferred.
- Valid driver's license and own transportation to make home visits.
Preferred:
- Good interpersonal and written and verbal communication skills.
- Physical, sensory, and cognitive abilities sufficient to perform essential functions.
