As an RN Case Manager Hospice, you will perform patient assessments, collaborate with the care team to develop and implement care plans, and provide education to patients and families. You will also manage patient care records, communicate regularly with the clinical manager, and perform on-call responsibilities as needed.
About You
Required:
- One year of nursing experience.
- Current RN license specific to the state of Georgia.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
