About the Role
As an RN Care Manager, you will report to the Supervisor of care management and be responsible for developing, implementing, and monitoring care plans in collaboration with primary care providers, caregivers, and other team members to meet the needs of members in the Medicare and Medicaid managed care health plans.
About You
Required:
- Associate’s Degree in Nursing for experienced candidates.
- BSN for new graduates.
- NYS RN license required.
Preferred:
- CCM certification preferred.
- 1 year of RN experience in managed care, home care, acute care, or nursing home preferred.
