About the Role
As a Registered Nurse, you will be responsible for managing patient needs through a team-based approach. Your duties will include care coordination, advocacy, and education for patients at risk of hospital admissions. You will assess and support patients, document care, and collaborate with the healthcare team to ensure high-quality care for chronic conditions and preventative measures.
About You
Required:
- Graduate of an approved school of nursing with RN license to practice in NYS.
- 2-3 years’ experience in Primary Care, Internal Medicine/Family Medicine, Public Health, or Case Management.
Preferred:
- Current certification in Basic Life Support for Healthcare Providers or equivalent.
- Bilingual skills.
