About the Role
As a PRN Hospice Social Worker, you will conduct thorough psychosocial assessments, provide individual and family counseling, facilitate advance care planning discussions, and connect patients with community resources. You will also offer bereavement support and follow-up care, ensuring that patients' wishes are honored at the end of life.
About You
Required:
- Master's degree in Social Work (MSW) from an accredited program.
- Licensure in the state of Indiana.
- Minimum of one year of experience in social work, counseling, or related in a healthcare field, preferably in hospice or palliative care.
Preferred:
- Multi-state licensure in Ohio, Illinois, and/or Texas.
- Strong understanding of grief and loss, psychosocial dynamics, and family systems theory.
- Excellent communication, interpersonal, and counseling skills.
