About the Role
As a Pharmacist in the Northern Network, you will provide comprehensive medication management for chronic diseases, conduct patient assessments, manage anticoagulation therapy, and collaborate with healthcare teams to improve patient outcomes. You will also educate patients and caregivers, document clinical interventions, and participate in quality improvement initiatives.
About You
Required:
- Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution.
- Completion of a PGY1 Pharmacy Practice Residency (PGY2 in ambulatory care preferred).
- Current pharmacist license in Michigan.
- Certification in Anticoagulation Management or equivalent experience.
- Strong clinical knowledge and communication skills.
- Proficiency with EHR systems and clinical documentation.
Preferred:
- Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS).
- Experience working with and utilizing a collaborative practice agreement setting.
