About the Role
As a Pediatric Community Health Worker, you will work closely with care teams and families to help patients navigate health and mental health care systems. Your responsibilities will include advocating for families, mapping community resources, connecting families with services, and assisting with applications for state resources. You will manage a caseload of patients and collaborate with various stakeholders to ensure comprehensive care.
About You
Required:
- High school diploma or equivalent required.
- 1-2 years prior experience working with children and families dealing with complex medical and psychiatric/behavioral problems.
Preferred:
- Associates/Bachelors degree in related field preferred.
- Bilingual English / Spanish highly preferred with fluency to speak, read and write.
- Exceptional communication skills and ability to multi-task.
- Ability to properly oversee up to 30 cases.
