About the Role
As an Outpatient Primary Care Physician, you will be responsible for delivering comprehensive care to senior patients, ensuring continuity of care, and addressing both medical and social needs. You will work in state-of-the-art centers with small patient panels, allowing you to build strong relationships with your patients. The position emphasizes quality over volume, with a supportive care team to assist in providing exceptional care.
About You
Required:
- M.D. or D.O. degree.
- Board certified/eligible in Internal Medicine or Family Medicine.
- Active, non-probationary state medical license.
Preferred:
- Fellowship training in Geriatrics.
- Experience in leading care teams with a patient-centric orientation.
- Ability to care for patients with comorbid conditions and those who are medically underserved.
