Summary
GuideStar Eldercare is seeking a Nurse Practitioner to join their team in New Braunfels, TX. The role involves addressing the needs of an aging population with neurocognitive disorders in nursing homes and assisted living facilities.
About the Role
As a Nurse Practitioner, you will travel to assigned facilities to provide initial evaluations, continuing therapy services, and cognitive testing. You will document the need and extent of services for each patient and maintain positive professional relationships with client facilities, patients, and their families, all while collaborating with a multidisciplinary team to promote patient safety and dignity.
About You
Required:
- Proof of Nurse Practitioner licensure.
- Ability to work as part of an interdisciplinary treatment team.
- Good time management skills.
- Able to function autonomously with regional Area Manager support.
- Pass pre-employment background check and drug screening.
- Complete credentialing process.
Preferred:
- Bilingual in English and Spanish.
