About the Role
As a Nurse Practitioner, you will be responsible for conducting comprehensive histories and physical exams, evaluating social determinants, and delivering health education. You will also order diagnostics, develop and adjust treatment plans, manage medications, and educate patients on disease prevention and self-care. The position offers flexibility for both full-time and part-time schedules, with a commitment to a minimum of 30 assessments per month.
About You
Required:
- Nurse Practitioner or Physician Assistant License in the state of New York
- Minimum 1 year of NP/PA experience
- Ability to commit to 30 assessments within 1 month
Preferred:
- Previous home care experience
