About the Role
As a Nurse Care Coordinator - Outreach & Wound Care, you will assess, treat, and educate patients on wound management while coordinating care between clinic-based and community-based services. You will monitor healing progress, develop individualized treatment plans, and ensure continuity of care for your caseload. This position requires both in-office and field-based work, including outreach to underserved populations and participation in community programs.
About You
Required:
- High School Diploma or equivalent.
- Active LPN or RN license in good standing (Delaware or compact state).
- Minimum 3 years of nursing experience, with at least 1 year in wound care or community health.
Preferred:
- Associate's or higher in Nursing.
- Wound Care Certification (WCC) or equivalent credential.
