About the Role
As an LTSS Service Coordinator, you will be responsible for facilitating the Person Centered Planning process, conducting assessments, developing comprehensive support plans, and managing the physical and behavioral health needs of individuals. You will also coordinate care with interdisciplinary teams, document service goals, and report critical incidents as necessary.
About You
Required:
- BA/BS degree and a minimum of 2 years of experience working with a social work agency.
- Ability to perform face-to-face program assessments and apply motivational interviewing techniques.
Preferred:
- BA/BS degree in a health care related field.
- Experience in mentoring or training new staff.
