About the Role
As an LTSS Service Coordinator - Clinician, you will be responsible for assisting the RN in identifying high-risk members, collecting clinical data, and coordinating care for members with chronic illnesses. You will participate in the development and monitoring of care plans and ensure efficient utilization of health benefits.
About You
Required:
- LPN/LVN, LSW, LCSW, LMSW, or other applicable license as per state law.
- Minimum of 2 years of experience working with individuals with chronic illnesses or disabilities.
Preferred:
- MA/MS in Health/Nursing.
- Experience in a Service Coordinator or similar role.
