About the Role
As a Licensed Social Worker, you will be responsible for facilitating admissions and discharges, developing discharge plans, providing family support, assessing participants for mental health issues, and advocating for participants' rights. You will also coordinate additional support services and participate in quality assurance evaluations.
About You
Required:
- LMSW/LSW is required.
- Experience with geriatric patients or in a Skilled Nursing Facility.
Preferred:
- Successful completion of a training and competency evaluation program.
- Completion of orientation required by company policy.
