About the Role
As a Home Health LPN, you will provide home-based care to patients with various health conditions, collaborate with healthcare teams, take patient vitals, document progress, and educate patients and families on care plans. You will work under the supervision of an RN and have the autonomy to manage your schedule.
About You
Required:
- Graduate of an accredited practical nursing program and active LPN license in PA.
- Minimum of 1 year of LPN experience in home healthcare, or completion of office’s preceptor program.
- CPR certification.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing, strong communication skills and compassion.
