Summary
Elevance Health is seeking an IDD Specialty Care Manager to join their team in Charlotte, NC. The role involves coordinating operations and workflows related to Intellectual, Developmental & Disability case management activities in support of the Children and Families Specialty Plan.
About the Role
As an IDD Specialty Care Manager, you will engage collaboratively with key stakeholders to craft individualized treatment plans, conduct assessments, develop comprehensive care plans, and coordinate with internal and external resources to meet the needs of members. You will also manage complex cases, provide support to Special Programs case managers, and ensure compliance with departmental standards.
About You
Required:
- MS/MA in social work, counseling, or a related behavioral health field or a degree in nursing with a minimum of 7 years of clinical experience in social work counseling.
- Active, current and valid license as an RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist.
Preferred:
- Experience working with Children, Youth, and Families served by Local Departments of Social Services.
- Knowledge of resources and services for safe community living for populations receiving in-reach and transition services.
- Clinical experience in social work counseling with complex psychiatric/substance abuse disorders.
- Certified Case Manager Certification.
