Inspire Hospice and Palliative Care is seeking a Hospice RN Case Manager to join their team in Kennesaw, GA. The role involves providing compassionate care and support to patients and families during their end-of-life journey.
About the Role
As a Hospice RN Case Manager, you will be responsible for being a steady source of comfort and support to patients and families, helping them understand disease progression, self-care, and nutrition. You will collaborate with the interdisciplinary hospice team to create and follow patient care plans, manage symptoms, advocate for patient comfort, and maintain timely and accurate clinical documentation.
About You
Required:
- Valid GA RN license
- Driver's license
- At least one year of experience in hospice or home health care
Preferred:
- Experience in managing end-of-life care
- Certification in hospice and palliative nursing (CHPN)
