Upham's Community Care is seeking a Health Aide-PACE Center to join their team in Boston, MA. The role involves assisting in the daily operation of the Adult Day Health Center and providing care to its participants.

About the Role

As a Health Aide, you will be responsible for personal care, restorative therapy, nursing assistance, and recreational activities for participants. You will work closely with the clinical team to implement care plans, assist with daily living activities, and participate in team meetings. The position requires flexibility to take on clinical and administrative tasks as needed.

About You

Required:

High School graduate or equivalent diploma such as GED.
Current certification as Home Health Aide, Nursing Assistant, or Medical Assistant from an approved program.
One or more years of experience working with the elderly in a health care setting.

Preferred:

Flexibility, initiative, creativity, and good interpersonal skills.
Physical capability to lift and provide maximum assistance with transfers.
Ability to speak Spanish, Portuguese Creole, or French Creole.
