About the Role
As an HCS Service Coordinator, you will assess individual needs, develop and monitor Person Directed Plans (PDP), and connect individuals with necessary services and supports. You will work under moderate supervision, ensuring compliance with federal, state, and local guidelines while advocating for individuals' rights and maintaining positive relationships with clients, families, and service providers.
About You
Required:
- Bachelor's degree from an accredited college or university in social, behavioral, or human service field.
- A minimum of one year's experience in the human services field.
Preferred:
- Experience in MHMR or Service Coordination.
- Knowledge of rehabilitation programs for individuals with developmental disabilities.
Benefits
- Competitive hourly pay ranging from $21 to $22.
- Full-time, non-exempt position with a standard work schedule of Monday through Friday, 8AM-5PM.
