ADAPT Community Network is seeking a Habilitation Specialist to join their team in Bronx, NY. The role involves designing and implementing individualized programs to enhance community participation for individuals with developmental disabilities.
About the Role
As a Habilitation Specialist, you will serve as a liaison between the agency, the individuals supported, and their families. Your responsibilities will include developing and implementing day habilitation plans, supervising staff, maintaining records, and utilizing community resources for program integration. You will also participate in training and orienting support staff, plan activities, and provide direct care as needed.
About You
Required:
- Bachelor's Degree in a Human Services or related field.
- One year of post-graduate experience is required.
- Ability to read, write, speak and understand English.
- Good oral and written communication skills.
- Good interpersonal skills necessary to interact effectively with co-workers, employees, the people we support, and their families.
Preferred:
- Valid New York State Driver’s License.
- Good organizational and teaching skills.
- Proficiency in the use of a personal computer and appropriate software.
- Ability to operate wheelchairs and adaptive equipment utilized by persons served.
