About the Role
As a Family Practice Provider, you will manage chronic and acute conditions, perform routine physicals and wellness visits, develop treatment plans, and coordinate care with a supportive in-house team. You will see an average of 20 patients per day, ages 18 to 90, and must have diagnostic and prescribing authority. EMR fluency and a proactive, patient-centered approach are essential.
About You
Required:
- Board-certified or board-eligible MDs or DOs, or licensed Nurse Practitioners (FNP preferred).
- Must hold an active DEA license and be autonomous.
- Must hold or be eligible for Illinois licensure.
Preferred:
- Experience or interest in endocrinology and/or functional medicine.
- Personable, adaptable, and eager to grow with the practice.
