Trinity Health is seeking a Community Health Worker -- Homeless Outreach to join their team at Mercy Medical Center in Springfield, MA. The role involves assisting homeless individuals in accessing social services and healthcare to improve their health and lives.
About the Role
As a Community Health Worker, you will improve access to health and human services for the homeless population through in-person interactions and coordination of referrals. You will facilitate enrollment in federal, state, and local programs, maintain contact with community agencies, provide individual and community support, and assist with transportation needs.
About You
Required:
- CHW Level I- Minimum high school diploma or equivalent.
- CHW Level II- Minimum Bachelor's Degree in human services or related field.
- Basic Life Support (BLS) certification.
- Current Driver's License in good standing.
Preferred:
- Bilingual (Spanish/English) strongly preferred.
