About the Role
As a Community Health Worker, you will connect individuals to health and social services, conduct outreach, coordinate care, and promote community engagement. Your responsibilities will include providing health education, bridging gaps between communities and services, assisting clients in accessing necessary services, and advocating for individual and community needs.
About You
Required:
- High School diploma or equivalent required.
- 1 to 2 years of experience working in a community-based setting.
- Bilingual (English/Spanish) required.
- Excellent oral and written communication skills.
- Knowledge of community social services and resources.
- Basic computer skills.
Preferred:
- Successful completion of a CHW training program.
- CHW certification preferred.
