About the Role
As a Clinical Pharmacist, you will be responsible for optimizing medication therapy to improve clinical outcomes, managing pharmacist consults for chronic care management, and collaborating with providers and interdisciplinary care teams. You will also provide patient-centered education, participate in quality improvement programs, and serve as a drug information resource.
About You
Required:
- Pharm D. degree or equivalent education in pharmacy
- Active, unrestricted Pharmacy license in Texas, or the ability to obtain one
- 3+ years of clinical pharmacist experience or equivalent residency experience
- Advanced in Microsoft Office platforms including Word, Excel, and PowerPoint
Preferred:
- Bilingual/Bicultural (English and Spanish)
