The Center for Alternative Sentencing and Employment Services (CASES) is seeking a Case Manager for their Outpatient Mental Health Clinic/CCBHC in New York, NY. The role involves providing core case management services, including care coordination and health promotion for clients with mental health and substance use needs.
About the Role
As a Case Manager, you will work as part of a multidisciplinary care team to provide targeted case management services to clients. Your responsibilities will include engaging clients in a collaborative relationship, conducting screenings and assessments, supporting clients with urgent needs, and providing referrals to community resources. You will also conduct community outreach and ensure clients have access to necessary supports, all while maintaining a person-centered and culturally sensitive approach.
About You
Required:
New York Certified Peer Specialist (NYCPS), Credentialed Family Peer Advocate (FPA-C) or Credentialed Youth Peer Advocate (YPA-C) with 3 years of work experience.
Bachelor’s degree in human services, social work, psychology, rehabilitation or a related field.
2 years of experience working with people with serious mental illness/substance use disorders.
Preferred:
Fluency in Spanish.
Individuals with lived experience in the areas of mental health, substance use, or criminal legal are encouraged to apply.
