Summary
Axis Health System is seeking a Case Manager to join their team in Durango, CO. The role involves supporting patients in the Jail Transitions program by initiating and continuing services for individuals with mental health disorders.
About the Role
As a Case Manager, you will be part of a multidisciplinary team that assists individuals diagnosed with or being assessed for mental health disorders. Your responsibilities will include care coordination, service initiation while incarcerated, and ensuring a smooth transition to community services. You will work closely with patients to help them access medical, social, educational, and other necessary services.
About You
Required:
- Bachelor's degree in a recognized mental health profession.
- Knowledge of patient health information and HIPAA compliance.
- Previous experience with substance abuse and mental health case management.
Preferred:
- Master's degree in a recognized mental health profession.
- One year or more of human service-related experience.
