About the Role
As a Case Management Coordinator, you will engage with individuals in a welcoming and empathetic manner, assess their needs, and develop personalized treatment plans. You will screen for co-occurring disorders, advocate for clients, and collaborate with community resources to provide necessary support services.
About You
Required:
- Must be a CMHP in accordance with Medicaid Provider Manual Guidelines.
- Possess a Bachelor's degree with three years of supervised experience in the examination, evaluation, and treatment of minors and their families, or be a Master’s prepared LLMSW or LMSW with one year of relevant experience.
- Must possess a valid motor vehicle operator's or chauffeur's license.
