Summary

About the Role
As a Care Transition Navigator, you will cultivate and nurture relationships with referral sources, collaborate with facility discharge planners, interact directly with patients and families, facilitate communication between care team members, and deliver exemplary care along the care continuum. You will also support a strong team culture and consistently meet individual performance goals.
About You
Required:
- Active and unencumbered license as a Registered Nurse (RN), Licensed Vocational Nurse (LVN), or Physical Therapist (PT) in the state of practice.
- Commitment to delivering compassionate, patient-centered care.
Preferred:
- Previous experience in home health, hospice, or healthcare setting.
- Strong communication and coordination skills across interdisciplinary teams.
