Summary
Community House Mental Health Agency is seeking an AOT Case Manager to join their team in Seattle, WA. The role involves providing intensive outreach-based wraparound services for individuals in the Assisted Outpatient Treatment program.
About the Role
As an AOT Case Manager, you will work as part of a team to provide outpatient mental health services and community support. Your responsibilities will include writing treatment plans, conducting individual treatment sessions, facilitating group therapy, and advocating for member needs. You will also perform outreach to assess living situations and provide necessary interventions, while maintaining confidentiality and collaborating with the clinical team.
About You
Required:
- Bachelor’s degree in social science or related field.
- Minimum 6 months of clinical experience working with severe mental health and/or substance use disorder individuals.
- Current AAC registration or ability to apply within 30 days of hire.
- Current CPR/First Aid certification or ability to obtain within 30 days of hire.
- Valid WA State driver’s license for 1 year with a clean driving record.
Preferred:
- Prior experience with mental health population.
- Ability to perform duties outlined above within reasonable accommodation.
