Heart of Hospice is seeking an Afterhours Registered Nurse to join their team in Baton Rouge, LA. The role involves providing comprehensive hospice care for patients and their families after regular business hours.
About the Role
As an Afterhours Registered Nurse, you will be responsible for triaging patient needs over the phone, responding to crisis calls, conducting initial nursing evaluations for hospice eligibility, and performing afterhours visits. You will also identify and reassess the physical, psychosocial, emotional, and environmental needs of patients and families, documenting all relevant information.
About You
Required:
- Current RN licensure in the state of practice.
- One year of clinical experience.
- Current CPR Certification.
- Current driver's license and access to a dependable vehicle or public transportation.
Preferred:
- Two years of full-time experience working as an RN in Louisiana, or two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted.
