Momentum for Health is seeking an Addictions Counselor I/II to join their team in San Jose, CA. The role involves providing substance use counseling and case management services to clients and their families.
About the Role
As an Addictions Counselor I/II, you will be responsible for facilitating one-on-one sessions and group counseling, assisting clients with social integration, coordinating care with medical professionals, and maintaining clinical documentation. You will work with clients to develop relapse prevention and coping skills, and provide support during recovery transitions.
About You
Required:
- High School diploma, GED, or equivalent education plus three years’ experience; OR Associate’s degree plus two years’ experience; OR Bachelor’s degree/Master’s degree plus one year’s experience working in an addictions/substance use service setting.
- Active and current Registration or certification with a certifying organization approved by DHCS to provide substance use treatment such as CADC I, CADC II or CADC III.
- Demonstrated ability to work independently and in a small team environment.
- Engage and communicate effectively with others in both verbal and written English.
Preferred:
- Knowledge of Drug Medi-Cal Organized Delivery System (DMC-ODS), Mental Health Services Act (MHSA) and Wellness Recovery Action Plan (WRAP).
- Familiarity with UNIX-based databases.
